Why can my child know what to do and still not do it?
A parent’s guide to executive function, task performance and the practical gap between understanding an instruction and carrying it out.

The short answer
Knowing a task and carrying it out draw on different processes. The child may need to notice the cue, stop another activity, hold the goal in mind, choose a first action, organise materials, estimate time, resist competing attention, monitor progress and recognise when the task is finished. A difficulty at any point can look like refusal, laziness or lack of motivation. Executive function is an umbrella for several of these processes, not one ability that a child either has or lacks, and it does not explain every unfinished task. Check understanding, skill, sensory and emotional load, sleep, health, task meaning and the child's account. Then move one demand out of the child's head: show the first step, prepare the materials, make time visible or work nearby. Keep the outcome clear while testing what makes action possible.
- Understanding an instruction does not guarantee access to every process needed to carry it out.
- Executive function covers several related processes and can vary by task, setting, development and current capacity.
- An unfinished task does not reveal whether the child did not care, chose not to act or could not organise action.
- Check skill, communication, sensory load, anxiety, sleep, health and environment before settling on an executive explanation.
- External supports can increase independence by making information and cues available at the point of action.
- Hold necessary boundaries while changing timing, method, prompting and support.
Your child can explain exactly how to get ready for school. They know where the uniform belongs, what goes in the bag and when the family leaves. Twenty minutes later, they are sitting on the floor with one sock on, absorbed in something they found under the bed.
It is easy to conclude that the knowledge was not genuine, the instruction did not matter or the child chose not to cooperate. Sometimes a child does make a choice an adult dislikes. Sometimes the task was unclear or unwanted. On other occasions, the child knows the outcome and cannot organise the sequence of actions needed to reach it.
The distinction is not visible from the unfinished task alone.
That uncertainty deserves careful attention.
Knowing and doing are different jobs
Knowing may mean the child can describe the task in a calm conversation. Doing it at 7:30 on a school morning can require them to:
- notice that it is time to begin;
- disengage from the current thought or activity;
- hold the overall goal in mind;
- identify which action comes first;
- find and organise the materials;
- ignore or recover from interruptions;
- estimate how long each part will take;
- notice whether the plan is working;
- tolerate boredom, uncertainty or fear of error; and
- recognise when the job is complete.
These demands are often grouped under executive function. The term is useful because it moves the conversation beyond knowledge and obedience. It can become misleading if adults treat it as one control system that works or fails as a unit.
A child may hold one step in mind and struggle to plan five. They may begin easily beside another person and drift when alone. They may organise familiar craft equipment and have no route into an open-ended history project. The pattern is the information.
Executive function is not a diagnosis
Executive-function differences are discussed in ADHD, autism and other developmental, learning and mental-health contexts. They also change with age, sleep, stress, pain, interest, familiarity and the environment.
A 2023 systematic review and meta-analysis compared children and adolescents with ADHD and autism across attention, working memory, inhibition, flexibility, planning, processing speed and related tasks. Both diagnostic groups showed average differences from comparison groups in several areas, while direct ADHD-autism differences were not clear across the measures [3]. The studies used laboratory or clinical tasks and varied methods. They cannot tell a family which process stopped one morning routine.
Research within ADHD also shows heterogeneity. In one detailed study of 136 children, patterns differed across working memory, inhibition and shifting rather than producing one uniform profile [4]. A single test result, checklist or everyday failure cannot diagnose ADHD, autism or an executive-function disorder.
Use the language to form questions, not a conclusion about the child.
Find the point where action stops
Choose one real example and reconstruct it without judgement.
The outcome: finish packing the school bag.
What happened: the child found the books, could not decide whether the PE kit was needed, checked the timetable on an app, saw a message and never returned.
The stoppage may be:
- uncertainty about what belongs in the bag;
- no visible sequence for checking;
- a distracting route through the phone;
- materials spread across rooms;
- difficulty returning after interruption; or
- anxiety about discovering that work is missing.
"Poor organisation" is too broad to design from. "The only timetable is inside a distracting app" gives the family something to change.
Ask the child what happened. Their account may reveal a different point: "I knew the kit was needed, but it was wet" or "I stopped because the worksheet was not finished." Do not make the executive explanation swallow an ordinary practical problem.
Check whether the instruction arrived
A child cannot act on information they did not hear, understand or connect with the situation. Before focusing on memory or motivation, check:
- whether attention shifted to the speaker;
- whether the words were clear and concrete;
- whether background sound or activity masked them;
- whether the child understood the vocabulary;
- whether the instruction contained hidden steps; and
- whether the child can show the first action.
NHS guidance for ADHD support includes clear, simple instructions given one at a time, visible to-do lists and shorter task periods [2]. These are options to test, not a formula that works for every child.
Ask, "Which part reached you?" rather than "What did I just say?" The first checks communication. The second can turn recall into an examination.

Motivation is not a quantity a child possesses
"They have no motivation" can describe several different states:
- the outcome has little value to the child;
- the benefit or consequence is too distant to affect action now;
- the task is unclear, too large or associated with failure;
- beginning requires more effort than the child can currently organise;
- the activity they must leave is more immediate or absorbing;
- anxiety makes avoidance feel safer;
- fatigue, low mood or pain has reduced capacity; or
- the child is choosing not to comply.
Each calls for a different response. A reward may make the outcome closer but will not teach an unknown skill, find missing materials or make a sensory barrier disappear. Repeating why the task matters tomorrow may not create a first action now.
Ask what would need to change for the child to begin. Keep open the possibility that the answer concerns skill, fear, fairness or relationship rather than executive function.
Ability in one activity does not settle another
Parents often hear, "They can focus on games for hours, so they can focus on homework." Sustained attention is not equally available across tasks. Games and chosen interests may provide immediate feedback, clear rules, visible progress, novelty, repeated attempts and a strong reason to continue. Homework may be ambiguous, delayed in its benefit and closely associated with correction.
That does not prove that intense focus is caused by ADHD or that the child is unable to stop. It shows that the two activities place different demands on attention, initiation and switching.

Move one demand into the environment
An external support holds or signals something the child otherwise has to generate internally.

NICE defines environmental modifications as changes that reduce the day-to-day effect of ADHD and includes written reinforcement of spoken requests, reduced distractions and shorter periods of focus among possible examples [1]. The right change remains individual and should be reviewed.
Start with one point of failure. Covering every wall with charts can add visual information without making the required cue easier to find.
Independence can include tools and people
Adults use calendars, alarms, satnav, written agendas and colleagues. A child who checks a list without an adult repeating it has acted independently through a tool. A teenager who asks for written instructions has used a self-advocacy skill.
The phrase external brain describes these arrangements; it is not a clinical intervention. The FamilyFX module on building an external brain shows how to move steps, time and materials closer to action.
Another person's presence can also be support. A parent may fold laundry nearby while the child starts homework, or sit silently while a teenager sorts a bag. The aim is not surveillance. Agree what the person will do, what they will not do and when they will leave.
Reduce support when the child can retrieve the next action another way. Keep it when it remains useful. Removing a tool to test whether the skill was learned without support can recreate the barrier without adding knowledge.
Keep necessary boundaries clear
An executive explanation does not remove limits around safety, consent or shared household needs.
The food needs to go back in the fridge tonight. You can use the three-step card, do it beside me or ask me to give one step at a time.
The outcome is fixed; method and support remain flexible. If the task can wait, say so. False urgency makes future cues harder to trust.
When the child does not act, describe the result without deciding intent:
The bag was not ready, so we had to return for the book. Tonight we are moving the timetable out of the phone and putting the checking list beside the bag.
The child may still need to repair an effect, contribute to the task or accept a limit. Accurate explanation and accountability can coexist.
Review the support against real outcomes
After several attempts, ask:
- Did the child begin closer to the agreed time?
- Could they find the first step without another explanation?
- Did they return after distraction?
- Was the task finished to a clear enough standard?
- Did adult prompting and conflict reduce?
- Can the child describe what helped or hindered?
- Is the system sustainable on an ordinary hard day?
If a support works only when a parent remembers to prepare, point, prompt and praise at every stage, the adult may still be carrying the whole system. That can be appropriate while teaching or during reduced capacity, but name it honestly and look for the next retrievable cue.
Look beyond executive function when the pattern changes
Sudden difficulty beginning, remembering or organising can reflect illness, medication effects, sleep loss, anxiety, depression, pain, bullying or a major change in circumstances. A child who used to manage and now cannot needs curiosity about the change, not a stronger planner.
Persistent difficulties across home and school can be discussed with teachers, the GP or the child's clinical team. NICE recommends a full clinical and developmental assessment for ADHD and consideration of coexisting needs, circumstances and physical health [1].
The practical question is not whether the child knows. Ask what the task requires between knowledge and completion, where that route breaks, and which part can be made easier to see, start or return to.
