GP and NHS Prep Pack
A short appointment is easier to use when the main question is clear and the relevant information is close at hand. This pack helps you prepare that information for a GP, referral conversation or neurodevelopmental assessment.
It cannot tell you whether your child is ADHD, autistic or has another condition. It does not produce a screening score or predict whether a referral will be accepted. Its job is smaller and more useful: to keep observations, sources and questions organised so you and the professional can decide what needs to happen next.
