Ask for the present support plan and check that it names the barrier, intended outcome, action, frequency, responsible person and review date. If the school proposes involvement from educational psychology, therapy or another service, ask what support continues while that conversation or referral is pending.

Keep education and health events in one chronology, but record the organisation responsible for each action. For an NHS referral, note the receiving team and date sent. For school support, keep the headteacher or named school contact visible so that a regional service name does not blur local responsibility.
