Ask the school to name the support, frequency, responsible person, intended result and review date. If an inclusion service is consulted, request a clear account of the advice given and how it will change classroom or whole-school practice.

Keep NHS referrals separate by receiving team and date. If the service is based in another part of Lanarkshire, consider travel and time away from school. Where education and health staff both contribute, ask who will coordinate the next review and circulate the agreed actions.
