Ask the school to record the barrier, support, frequency, responsible person and review date. If a council specialist or health professional is involved, clarify the question they are being asked and how their advice will be used in everyday classroom practice.

Keep the school contact and receiving NHS team separate in your record. A regional health-board name is not enough to track an individual referral. Note the service, referral date and next expected contact, then keep reasonable school adjustments moving while the health process continues.
