Practical next steps

Ask the school to name the present level of support and then translate it into actions: frequency, staff role, setting, intended result and review date. Where several services attend a meeting, ask for one action list that separates each organisation's responsibility.

Travel can determine whether a plan is sustainable. Check the location and timing of support, any school-transport assessment and what happens if weather or distance disrupts attendance. For health referrals, record the provider and team that received the referral rather than contacting the health board for an individual waiting-list update.
