Ask for the present school response in writing before focusing on future provision. Check actions, frequency, staff responsibility, intended outcomes and review dates. If a panel or specialist assessment is mentioned, ask what it decides and what information it requires.

Consider transport and the length of the day when discussing provision outside the local school. For health referrals, record the named receiving service and location. Keep education and NHS information in one chronology while making each organisation's responsibility explicit.
