Practical next steps

Ask the school to show how identified barriers appear in the current plan and what support follows at each part of the day. Check the frequency, responsible staff, intended outcomes and review date. If a multi-agency meeting is proposed, ask who is coordinating the actions afterwards.

In a large system, two services can sound similar while doing different jobs. Confirm whether each contact provides advice, assessment, treatment, education support or family help. Keep health and education timelines together, but do not let one service assume the other is taking action without a named handover.
