Ask for each agreed adjustment to be described in practical terms. Check who is responsible, where it applies, how often it should happen and when the school will review it with the child and family.

If educational psychology or an NHS team is consulted, clarify the purpose of the involvement and how recommendations will reach the people working with the child. Record the receiving health service rather than only NHS Ayrshire and Arran, and include travel or appointment demands when planning the school week.
