Ask the school to confirm its decision and the evidence it used. If an IDP is prepared, check that it describes the provision clearly, records what is important to and for the learner, and names who will deliver each action.

If you disagree, narrow the issue to the exact decision or part of the plan that needs reconsideration. Keep copies of the request and reply. For health involvement, record the service or hub contacted, what information was supplied and what happens next. Bring relevant advice to the education review rather than assuming it will be shared automatically.
