Practical next steps

Ask the school to record the barrier, the provision being tried and the evidence it will use at review. If an IDP is needed, check that it names the additional learning provision, who will deliver it and how often. Ask how specialist advice will be translated into daily classroom practice.

With several possible clinic sites, record the exact service and appointment location, not only the health board name. Keep a single chronology of requests, replies and review dates. If responsibility is unclear, ask each organisation to state what it will do next and what information it needs from the other.
