Ask the education setting to record the barrier, action, responsible person and review date. Where Welsh is the child's language of education or communication, make the language needed for assessment, planning and provision explicit in the conversation.
For specialist or health appointments, include travel time and the effect of a long day in the plan. Ask how recommendations will be shared with staff and used between appointments. If several services are involved, identify one person who will keep the next actions visible to the family.
