How support is organised in Torfaen

When a learner is struggling, the first conversation should make room for what matters to them as well as what adults have noticed. Some children can explain this directly; others may communicate through behaviour, drawings, choices or a trusted person.

Speak to the teacher, ALNCo or headteacher and take a few specific examples. Ask what can change now, how the learner will be involved and when the setting will review whether the support is working.

Torfaen County Borough Council's school guidance describes person-centred planning meetings involving the learner, family, school staff and ALNCo. It explains that the discussion should consider what is important to and for the child, what is working and what is not working, and use that information in the learner's one-page profile and any IDP.

Aneurin Bevan University Health Board's Gwent Neurodiversity work provides a regional source of information, support pathways and early-help resources. Education and health may need to share evidence, but they remain responsible for their own decisions and actions.

Practical next steps

Before a person-centred meeting, help the learner choose how they want to contribute. They might attend for part of it, prepare something in advance or ask an adult to share their words. Write down the support being tried and the practical change it is meant to achieve.

If an IDP is prepared, check that the additional learning provision is precise enough to use and review. If health advice is relevant, ask how it will be reflected in daily support. Record the person responsible for every action and return to the learner's experience at the review.
