Practical next steps

Ask the setting to confirm the support, responsible person and review date in writing. If there is a question about ALN, ask who is making the decision and how the learner's views will be gathered. An IDP should say what additional learning provision will be delivered, not simply name a condition or service.

If a neurodevelopmental referral is being considered, check the health board's current referral information rather than relying on an old form. Keep the date, receiving team and purpose of the referral in your record. At review, bring education and health information together but keep each organisation's responsibility clear.
