How support is organised in Swansea

Practical next steps

Ask the setting to record the barrier, support action, responsible person and review date. If an IDP is prepared, check that the additional learning provision is described precisely and that the learner's views are represented in a way they can manage.

For NHS involvement, record the receiving service rather than only the health board. Ask what the referral is intended to clarify, how advice will be shared and what support remains in place during the wait. Keep each organisation's next action visible in one chronology.
