How support is organised in Caerphilly

Practical next steps

Ask the setting to record the barrier, agreed support, responsible person and review date. If the learner may have ALN requiring additional learning provision, ask who will make that decision and how the learner and family will contribute.

Before a transition, record the support that works, the situations that remain difficult and the person responsible for sharing information with the next setting. For health questions, follow the current Gwent pathway and note the receiving team. Keep the council, school and NHS actions separate enough that each can be followed up.
